Surgery Request

Please Read the Following

  • A $20 deposit is required per pet at the time the surgery is scheduled to reserve your appointment slot. This deposit is applied towards the balance for services the day of surgery. If the client does not show up or cancels less than 48 hours before the appointment, the client will forfeit the deposit.
  • Clients using a county voucher will still be required to pay a $20 deposit.
  • All Vouchers (issued through the Hillsborough County Pet Resource Center) must be given to the clinic during check in. If the voucher is not given during check in, the client will be charged the regular cost for surgery and required vaccines. The cost of the voucher is $10. The voucher includes the basic cost of the Spay or Neuter, a 1 year Rabies Vaccine and a 1 year Hillsborough County Rabies Tag. Any additional costs are not covered by the Voucher and must be paid by the client. Examples of additional costs could include but are not limited to: In heat or pregnancy fees, additional required vaccines, e-collars, prevention, etc.
  • All dogs are required to have a Rabies vaccination and a Distemper/Parvo (DA2PP) vaccination to have surgery at Animal Wellness Center. All cats are required to have a Rabies vaccination and a Feline Distemper (FVRCP) vaccination to have surgery at Animal Wellness Center.
  • These vaccines can be given the day of surgery. If your pet already has the required vaccines then you MUST bring proof of vaccines on or before the day of your surgery appointment. Vaccines must have been given by a licensed veterinarian. If records are not provided you will be asked to reschedule.
  • The recommended age for spay or neuter is 6 months of age or older.
  • All cats must be in separate carriers, they can not share a carrier. We have carriers for sale if needed. All feral/any cats unable to be handled by staff must come in a live humane trap.
  • Senior pets should be in overall good health and will be required to have bloodwork before surgery.
  • All pets are recommended to go home with an E-collar or surgery suit after surgery. Clients can bring their own E-Collars or surgery suits but they must be brought in the morning of appointment. We have them to purchase as well.
  • Please fill out the surgery request form for each pet you are trying to schedule.

Surgery requests are accepted Saturday through Tuesday each week. A member of our team will contact all requesters Wednesday through Friday each week to schedule the appointments. If you are unable to be reached or do not return the call the same day, your request will be discarded. Please be prepared to make a $20 deposit to schedule your appointment when you are called. If you do not hear from us within 2 weeks of requesting the appointment, please contact a member of our team at 813-752-2869.(Required)
